Take Talamge exit and head West toward Ukiah. Turn Left onto Airport Park Boulevard heading South. After the stop sign make a Left into Park Falls Plaza. The plaza is located behind Les Schwab and South of The Tractor Supply Company. Meadows Physical Therapy is located on the North end of Park Falls Plaza and our door faces the Tractor Supply Company. *HINT: Do not turn left into the plaza after you pass Les Schwab, Keep going straight.*
